This form is to be used if a member school’s student is participating in an event that is not sanctioned by the B.C.S.S.M.A.

This form was created by B.C.S.S.M.A to assist sponsor teachers of member schools a format for notification and approvals by parents for students to participate in events that are not sanctioned by the B.C.S.S.M.A. Parental forms that have been completed for the sanctioned events are not to be used at any other events and new Parental Consent forms for these other events must be completed for each of the non-sanctioned events.
